Biosign And Peconic Bay Medical Center Sign Agreement

Riverhead - Biosign Technologies, a company focused on the non-invasive monitoring of common health risks, and Peconic Bay Medical Center announced an agreement to implement the Healthanywhere solution for remote patient management.

The Peconic Bay Medical Center Home Health Services which provides nursing assistance with approximately 20,000 home visits each year, will undertake an assessment of remote patient monitoring (the application and processes) of acute post-discharge patients as well as long term chronic disease patients in the community. The initial focus will be on a "Health at Home" program for remote patient monitoring of patients with diabetes, cardiovascular disease (CHF), and respiratory disease(COPD) to support Peconic Bay's commitment to maximize the health, comfort, mobility and independence of each and every patient through direct skilled care and education.

Initially, PBMC-HHS will deploy short-term remote monitoring that will serve acute post-discharge patients. The long term goal for PBMC-HHS is to service patient's longer term needs and support them to self-manage their various conditions. The goal of this agreement is for PBMC-HHS and Healthanywhere to provide a sustainable Health at Home solution for short-term remote monitoring of post-discharge patients with a baseline of vital signs for participating patients and trends on the impact of remote monitoring, education and behavioral change initiatives within the program.
